This SOP covers building a native GoHighLevel lead form, mapping fields to CRM, tagging, adding to a pipeline stage via a workflow, and embedding on funnels/sites or externally.
Build a simple GoHighLevel lead form, map fields to CRM, auto-tag, and place new leads into a pipeline stage
Ideal outcome
Every submission creates/updates a contact with correct fields, applies the tag, and opens an opportunity in the right pipeline/stage
Why it matters
A standardized form + workflow reduces setup time and prevents routing errors for small-agency lead gen
Before you start
You need admin access to the client’s GoHighLevel sub-account. You need edit access to the target Funnel/Website pages or external CMS (if embedding externally). Refer to the SOP 276: GoHighLevel CRM Foundations (web version) for instructions on field creation, tagging, and integration mapping

36 headings
1Create and confirm CRM custom fields (settings)5
Visit GoHighLevel and sign in.
Go to "Settings" → "Custom Fields" and click "+ Add Field".
Choose the right field type (text, number, etc.). Click "Next".
Give it a clear, descriptive name. Set up the field preferences relevant to the business requirements. Click "Save".
Repeat for any additional contact custom fields needed.
2Build the lead form5
Go to "Sites" → "Forms" and click "+ Add Form".
Select "Start from Scratch" and click "Create".
Name it "Lead Form – [Client/Offer]" (Use a clear, standardized name so the form is easy to recognize later). Add "First Name", "Last Name", "Email", and "Phone".
Drag and drop a custom field element, if necessary.
Click "Save" in the form builder. You should now see the form in your "Forms" list with the name you assigned.
3Create submission workflow5
Go to "Automations" → "Workflows" and click "+ Create Workflow" and choose "Start From Scratch".
Rename the workflow as "Lead Form" and add a trigger "Form submitted". Set filter to "Form is" = "Lead Form – [Client/Offer]". Click "Save Trigger".
Add action "Add Contact Tag" and enter the agreed tag (create if missing under "Tag"). Click "Save Action".
Add another action "Create/Update Opportunity" and set it to the existing "Pipeline" and "Stage" that you want and "Status" to "Open". Optionally set "Opportunity Name" to "{{contact.first_name}} {{contact.last_name}} – Lead". Click "Save Action".
Click "Save", then toggle on "Publish" to activate the workflow. The workflow status should change to "Published". Newly submitted leads should now trigger the automation. Sometimes the first trigger may take a couple of minutes.
4Embed the lead form on GoHighLevel page5
If using a Funnel, go to "Sites" > "Funnels" and open the target funnel. Click "Edit".
Now, click on the "+" button on your top left and select "Forms And Surveys". Under "Add Existing", drag and drop "Add Existing Form" element onto the page.
Click on the existing form and select your form. The "Lead Form – [Client/Offer]" should appear by now. Once selected, the existing form will appear on the draft page.
Optional - you can edit the form as needed.
Click "Published" in the page editor, then set the correct domain and click "Save".
5[Optional] Embed lead form on external site (get embed code)5
Go to "Sites" → "Forms" and open your lead form.
Click "Integrate".
Copy the embed code. You’ll see options for a script embed (matches site styling) or an iframe embed (more isolated, less flexible).
Paste the code into the destination page’s HTML in your external CMS.
Publish the external page.
6Run live test and verify CRM updates5
Open the live page with the form and submit a test entry.
Confirm the thank-you page loads.
Go to "Contacts" and open the test contact.
Go to "Opportunities" and locate the new opportunity.
Return to "Contacts" and open the test contact again.
